Certification Maintenance by Credit Portfolio - Interactive Activity Forms in WORD format 

These Word format forms can be opened, edited and saved on your computer. Once they are completed you must print them out, get the required signatures and then place them into your portfolio. If you want to fill in your forms by hand please use the (PDF forms).


  • Download and save the activity form on your computer.
  • Open the form.  
  • Using the tab key, or your mouse, select the shadow box where you wish to enter text. 
  • The cells will automatically expand to accommodate your text.  
  • Once a page is full, the tables will move on to the next page as you type. The header will appear on each page. 
  • For the check boxes, select the appropriate one and toggle between checked/unchecked using either the spacebar or left click on your mouse. 
  • Do not forget to save your forms after entering data.  You should also make a backup copy.
